Mechanism of derangements of the medial semilunar cartilage and their management.

The rotation element-The key to the function of the knee joint, and to its derangements, is the rotation of the tibia on the femur which occurs on flexion and extension movements. The knee is not a simple hinge joint. The opening and shutting of the front of the joint in the act of flexion and extension involves the tibia in a winding course set by the configuration of the medial condyle of the femur (Fig. 1). As the tibia passes on the femur from the fully flexed to the fully extended position it descends and then slowly ascends the curves of the femoral condyle and rotates outwards. These movements are reversed as the tibia passes back on the femur to the fully flexed position. The movement of the tibial tubercle, which is easily observed, confirms this spiral action.
